Neither. The number after the R tells you the wheel size. So R20 is a 20in wheel.

No. The R20 in the size means that they are for 20 inch wheels. R17 would be 17 inch, R18 would be 18 inch, etc.

surely if the tyre is R20 then it needs a 20 inch rim?

Yep. The tire size you gave is in inches. First number is height, second number is width, and last number is wheel size. So it’s a 35in tall tire, that’s 12.5in wide, and it fits on a 20in wheel. I’m assuming you’re looking at some sort of off-road/tuck tire because that is such a large size wheel/tire.
